Corra Practitioner Agreement

CORRA PRACTITIONER AGREEMENT

This Practitioner Agreement (the “Agreement”) is entered into between Corra Inc., a Delaware corporation (“Corra,” “Company,” “We,” or “Us”) and the practitioner named on the signature page hereto (“Practitioner” or “You”) as of the date of first purchase (“Effective Date”). Company and Practitioner agree as follows:

 

By subscribing to Corra’s platform, you agree to the following terms:

 

  1. Subscription & Access

    • Upon registration, you will receive a unique practitioner code that grants patients access to Corra’s platform under your subscription.

    • The Basic Plan includes 14 active patient seats, while Unlimited and Ultimate Plans provide unrestricted patient access.

    • You may not publicly share or distribute your practitioner code. It must only be provided to active patients in your care.

    • Misuse of access codes, including unauthorized distribution, may result in suspension or termination of your Corra subscription.

  2. Practitioner Responsibilities

    • You acknowledge that Corra is a health-tracking software designed to assist with patient data organization and does not provide medical diagnosis or treatment.

    • You are responsible for ensuring that patient use aligns with appropriate health practices and any relevant regulations in your jurisdiction.

    • You agree to communicate with patients regarding their participation and assist them in integrating Corra into their care plans as needed.

  3. Term & Termination

    • Your subscription remains active on a monthly or annual basis, per your selected plan, and will auto-renew unless canceled.

    • You may cancel at any time through your Corra account dashboard; however, cancellations do not grant refunds for previously billed periods.

    • Corra reserves the right to suspend or terminate this agreement at any time, for any reason, at its sole discretion, including but not limited to:

      • ​Violation of Corra’s terms of use, such as misuse of access codes or unauthorized distribution.

      • Engaging in any activity that harms the integrity, reputation, or functionality of Corra’s platform.

      • Actions that create a conflict of interest or are deemed detrimental to Corra’s mission, business operations, or community.​

  4. Corra’s Role & Support

    • Corra will make reasonable efforts to provide technical support, promotional opportunities, and educational resources to help practitioners integrate the platform.

    • If your plan includes marketing support, Corra may feature your practice on its website or promotional materials at its discretion.

  5. Patient Access & Transition

    • If your subscription lapses or is terminated, affected patients will be notified and given an opportunity to:

      • Transfer to a personal subscription to maintain access.

      • Contact Corra for potential practitioner referrals if applicable.

  6. Data Security & Privacy Compliance

    • Corra follows strict privacy and security protocols to protect user data.

    • While Corra is not a HIPAA-covered entity, we implement industry-standard data security measures to safeguard patient information.

    • Practitioners agree to use Corra in compliance with any applicable data protection laws in their jurisdiction.

  7. Limitation of Liability

    • Corra is not responsible for medical decisions, patient outcomes, or reimbursement claims associated with using the platform.

    • Corra provides its software on an “as-is” basis and makes no guarantees regarding insurance reimbursement.

  8. Marketing & Promotional Use

    • By enrolling in Corra’s practitioner program, you grant Corra permission to use your name, practice name, and professional title for marketing and promotional purposes. This includes, but is not limited to:

      • Featuring your practice on Corra’s website, social media, and promotional materials.

      • Referencing your participation in Corra’s program during sales, marketing, and partnership discussions.

    • This permission does not obligate Corra to promote your practice, nor does it guarantee inclusion in any specific materials.

  9. Acceptance of Terms

    • By purchasing a Corra practitioner subscription, you acknowledge that you have read and agree to the terms of this Agreement.
